PARENT'S STATEMENT & RELEASE OF LIABILITY (All Camps)
 
The parent/guardian must give consent for the registered camper to participate in the activities of this camp, to include the specific sport activities and any other recreational activities conducted at the camp. The undersigned parent/guardian understands that the camper will engage in physical activity during the camp which contains some inherent risk of physical injury, and assumes all risk and releases William Carey University, its officers, Directors, Agents, and Employees as well as any assigned coaches or volunteers working for the camp from any and all liability for personal injury arising out of the camper's participation in the camp program.. If at any time it is necessary for the camper to receive outside or professional medical attention, the parent/guardian hereby gives consent to secure the services and arrange transportation to a medical facility if deemed necessary. It is understood that campers are held responsible for all medical expenses resulting from sickness, accidental injury or any other non camp-related injury or illness acquired at the time of the camp.
